Thanks to The Arizona Republic/12 News Season for Sharing for their generous $5000 grant in support of our Care Transitions Program!

This contribution will enable our Care Transitions Team to implement a pilot program to help older adults by providing much-needed support services immediately after their discharge from one of our local hospitals.
